Well basically they won't know unless something goes wrong. For example if you get pulled over in a rental you are required to provide the rental agreement, if the rental agreement does not permit you to drive out of province from what i've been told you may be detained and the vehicle towed at your expense because you are illegally driving the vehicle.
Also, if you say got in an accident out of province, then of course you have broken the contract, so if you paid for rental insurance through the rental company they will not cover you and if you are using your own personal insurance chances are they will not cover you because you voided the contract.
As for going into the states, you may be asked to provide documentation for the rental at the border and you won't get through with a contract only valid in canada.
On a related note, in the near future rentals will be equipped with GPS (for recovery purposes mainly) so they can track you if you go out of province.
