They will basically give you a refub on the spot for any issue, as for the "geniuses" they will basically just reiterate what you told them with "So if I understand correctly, _________ happened. Is this right?" and then give you the refurbished phone. As for earbuds you can get a replacement set in the same way, if not then you can call the AppleCare support line and talk to someone in Apple Customer Relations to get results. More often than not though, depending on what the issue is I'd recommend opting for a third party headset.
I know I helped a fellow a few days back who while working out, had his earbuds blast to maximum volume. The cause was that a drop of sweat had dripped down the cable and upon hitting the remote it shorted it out.
